Presenting Case
In this case study, we delve into the therapeutic journey of a middle-aged lady suffering from obsessive-compulsive disorder (OCD) and depression. Referred for a second or third opinion due to lack of improvement with previous treatments, her condition had taken a toll on various aspects of her life, including her marriage and physical health. Despite being labelled as “treatment resistant” by her family and previous psychiatrists, a steadfast therapeutic relationship developed, highlighting the importance of hope and perseverance in such cases.
Case Description
The patient, a middle-aged woman, presented with longstanding OCD and depression that had not responded to previous treatments. Her condition had significantly impacted her personal and social functioning, leading to distress within her family and exacerbating her physical health problems. Despite facing numerous challenges, including resistance to pharmacotherapy and psychotherapy, the patient’s determination and the collaborative efforts of the healthcare team were instrumental in achieving significant improvements in her symptoms and quality of life.

Diagnosis and Treatment
Diagnosing and treating this case presented several complexities. Pharmacotherapy, despite its potential benefits, encountered resistance due to concerns about medication tolerability and side effects. Similarly, psychotherapy, particularly Exposure Response Prevention (ERP), faced challenges in implementation due to the patient’s reluctance and underlying familial tensions. However, pharmacotherapy was eventually found to be effective, albeit with a medication regimen that is atypical from conventional recommendations. Uncovering and addressing these underlying issues required a multifaceted approach, incorporating both nuanced and interpersonal interventions tailored to the patient’s unique circumstances. This approach, coupled with the commitment of a dedicated healthcare team, proved instrumental in guiding the patient towards recovery.
Management Approach
A collaborative approach involving the healthcare team, the patient, and her family was paramount in navigating the complexities of this case. From the outset, establishing rapport and fostering open communication were essential in building trust and facilitating the patient’s active participation in treatment decisions. Treatment modalities were carefully selected based on a thorough assessment of the patient’s preferences, tolerability, and treatment goals. It is also important to consider a patientโs personality traits as they can heavily influence patient preference and the success of treatment strategies. Regular monitoring and adjustment of treatment strategies ensured that interventions remained tailored to the patient’s evolving needs, promoting sustained progress and engagement in therapy.

Discoveries and Considerations
Throughout the treatment journey, no unexpected discoveries or developments arose, allowing for a focused and consistent management approach. Instead, the focus remained on addressing the known challenges and barriers to treatment, with a steadfast commitment to achieving meaningful outcomes for the patient. Ethical dilemmas or decision-making processes did not arise during treatment. However, maintaining patient autonomy and respecting her preferences and values were central tenets of the therapeutic approach, ensuring that treatment decisions remained aligned with her individual needs and goals.

This case highlights the efficacy of personalised pharmacotherapy in challenging treatment-resistant cases, showcasing the importance of adapting medication regimens to individual patient needs. It also emphasised the significance of perseverance, collaboration, and patient-centred care in achieving positive outcomes. The enduring therapeutic relationship established with the patient, demonstrated by ongoing communication and support even after treatment, shows the profound impact of compassion, empathy, and personalised therapy towards promoting recovery.
