The “Healthier SG” initiative recognizes that primary care, delivered by family doctors, is the critical first touchpoint in a patient’s care journey. It emphasizes the need for strong patient-doctor relationships, as well as the management of chronic diseases, to be embedded within the community.

Strengthening Patient-Doctor Relationships

One of the core principles of “Healthier SG” is the building of strong, long-term relationships between Singaporeans and their family doctors. These relationships allow family doctors to understand their patients’ health conditions and circumstances, which, in turn, enhances health outcomes.

Expanding Primary Care Capacity

Over the last decade, significant investments have been made in primary care capacity. Initiatives like the Community Health Assist Scheme (CHAS), the Chronic Disease Management Programme (CDMP), and subsidies for recommended vaccinations have increased the involvement of family doctors in providing subsidized primary care.

The Role of Primary Care Networks (PCNs)

To enhance the delivery of primary care, the Ministry of Health has worked with individual practitioners in private GP clinics to form Primary Care Networks (PCNs). These networks provide essential services like chronic disease management, IT system upgrades, and nurse support.

“Healthier SG” aims to ensure consistent and well-coordinated care protocols for chronic diseases. This includes developing a Clinical Service Roadmap for Primary Care. The roadmap, developed in collaboration with healthcare clusters, medical associations, and PCNs, will initially focus on protocols for common chronic conditions such as diabetes mellitus, hypertension, and lipid disorders.

Supporting Lifestyle Adjustments

The initiative also encourages lifestyle adjustments through care protocols, including areas like smoking cessation and weight management. It recognizes that primary care physicians can play a significant role in guiding patients to healthier habits.

Fair Remuneration for Family Doctors

As “Healthier SG” shifts the focus towards preventive care and long-term relationships, family doctors in private practice will receive an annual service fee based on the health risk profile and scope of required care for each enrolled resident. This ensures that family doctors are fairly remunerated for their crucial role in maintaining patients’ health and well-being.

Encouraging Enrolment with Family Doctors

To further promote the patient-doctor relationship, steps will be taken to narrow the difference in drug subsidies between polyclinics and private clinics. This aims to make private family doctors a more financially attractive and convenient choice for patients seeking preventive care.
